CLCM MLP Release (English & Spanish)

This Release of Information allows the MLP attorney to speak with the school district on behalf of the student. Please fill out the name and DOB of the student but leave the school district blank in case it needs to be used with multiple school districts. The parent/guardian should sign the English version and retain the Spanish version for their own records.

Sample Evaluation Request

This letter should be used to request a special education evaluation. It can be signed by a parent/guardian/caregiver OR any person in a caregiving or professional position concerned with the student's development.
